LOCKED IN TRAINING ACADEMY LIABILITY WAIVER 

I hereby understand and acknowledge that the training, programs, and events held by Locked In Training Academy (the “Academy”)– either by my participation in-person or following trainers via video – may expose me to many inherent risks, including accidents, injury, illness, or even death. I hereby assume all risk of injuries associated with participation including, but not limited to: falls, trips, contact with other participants, trainers, coaches, and inanimate objects. I also understand that my participation in training, programs and events may bring about certain personal risks out of the Academy’s control – including but not limited to the weather, high heat, and/or humidity, and I hereby acknowledge the risks and agree that I am voluntarily participating in these activities and assume all risks of injury, illness or death. 

I certify that I am physically fit and there are no health-related reasons or problems which prohibit my participation. I have NOT been advised to abstain from participation in such activities by a qualified medical professional, and I certify that I have health insurance coverage. 

After having read this waiver and knowing these facts, and in consideration of acceptance of my participation in any training, programs, and events sponsored or recommended by the Academy. I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE the Academy, its employees, coaches, staff, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in the Academy’s activities, camp, workouts, programs, and training. 

I further agree to completely WAIVE MY RIGHT TO A TRIAL BY JURY upon any recognition of a cognizable claim under federal or state law, including but not limited to claims arising out of personal injury, negligence in tort, or breach of contract. Upon any legal disagreement, I hereby agree to submit to a neutral arbitration with a licensed arbitrator in the forum of Middlesex County, New Jersey, USA. Any dispute shall be decided by New Jersey law. 

If any portion of this waiver shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this waiver shall remain in full force and effect and the offending provision or provisions severed here from. 



By my signature I indicate that I have read and understand this Liability Waiver. I am aware that this is a wavier and a release of liability and I voluntarily agree to its terms.
